Documenting a Condition

Social Security disability lawyers equip their clients with forms (MSS - Medical Source Statements) for their doctors to complete which help establish the connection between the disabling condition and the resulting physical or mental limitations on ability to work. Although some doctors readily agree to fill out these forms, others don't (for a variety of reasons). It is always essential that patients describe with detail their symptoms/limitations at each visit and specifically to their doctor to help document limitations as much as possible. Strategies with a lawyer should be explored.
